eHealth - Telemedicor Guardian for Diabetes
What we offer:
Telemedicor Guardian is a simple to use but extremely powerful Telemedicine tool that allows diabetic patients to measure their blood glucose using their existing glucometer, and have the measurements sent to the doctor’s patient monitoring system by simply connecting the glucometer with the special Telemedicor Guardian GPRS modem.
Functioning as the ideal tool for doctors and healthcare professionals, whenever there are deviations from the parameters set by the supervising doctor for an individual patient, the system allows initiation of an automated Telemedicor Guardian web application response (e.g notification of the doctor via email or SMS). In this case; the doctor can recall instantly the entire diabetic patient’s readings in order to fully evaluate the current situation and contact the patient, if necessary, to commence corrective measures.
Telemedicor Guardian enables care givers to constitute treatments as a list of instructions, taking advantage of the currently available therapies and offers automated alerting methods. For example software constantly monitors whether a patient is sticking to his monitoring program. In case the patient stops measuring his blood glucose, or pays less attention to his self-monitoring schedule, an automatic predefined message can be send to him, stressing the need to continue to adhere diligently to his self-monitoring program.
With a simple graphical interface and tools for reporting & statistics works as a homogenized environment to diabetes patients tele-monitoring, no matter the type or the brand of the glucose meter the patient uses.
Moreover, Telemedicor Guardian gives the opportunity to each patient that has internet access, to view his own personal record only, as well as his physician’s instructions, as part of the complying methodology.
Telemedicor System’s state of the art consists on the followings:
1. Keeps the ICT application as simple as possible for the end users, both patients and Care Givers.
2. Respects their daily routines and needs. Thus they are less negative in accepting
changing their habits (behavioral changes).
Example: A patient simply needs to connect his existing blood glucose meter to Telemedicor GSM Modem for submitting data to his health record. Nothing additional.
No internet, No computer, No buttons to push, no menus to navigate, to more forgotten telephone calls. No high and complicate procedures to fear or to stress.
3. It remains simply interactive complying to a motivation strategy out of WHO directives.
Example: Communication process between patient-care givers turns to be proactive with the tool, with the physician always one step ahead from patients needs. A so called total quality process.
Whenever a patient submits data to his personal record, the care giver has the ability to send him or a member of his family (parents etc) an SMS or email with notifications, updates, instructions, remarks etc.
4.Turns patient as part of a “care system”. Thus he is not feeling alone in fighting with the disease.
His physician, his family members and technology, all together, support him and improve his compliance with the management of a chronic condition such as diabetes.
5. Considers care giver as “decision maker”. Thus it provides him with any information available in simplicity and within prompt time.
6. Respects care giver’s mobility. Accessing his patients’ records via the Telemedicor web based platform, enables the care giver to show more concern and increases the patient’s confidence in him.
